Making medication use safer and more effective
Medication management refers to the rational selection and use of drugs for a given patient who has complex or simple medical needs and conditions, so they can derive as much benefit as possible from their medications.
Safety considerations are paramount in this form of treatment, helping to prevent overdoses, harmful side effects and other medication-related problems.
Although medication management is important for anybody taking multiple medications on an ongoing basis, it can be particularly important for older people.
Where someone takes five or more different medications every day, this is referred to as ‘polypharmacy’. This can increase the risk of drug interactions or duplication. At the other extreme, doses that are too small, missed doses or incomplete courses of medication may reduce the intended effect of treatment.
